Repair Authorization Request Form

Please fill out this form as complete and as clearly as possible:

Note: If no defect is found, one (1) hour of labor will be charged @ $90.00/Hr.

All products have a ten-year warranty (except the HDE-3000 CC Encoder) on parts, and labor. All parts will be replaced under the ten-year warranty at no cost, including parts and labor. The warranty does not include obsolete parts. Link will make every attempt to obtain obsolete parts, so the product may be repaired. The HDE-3000 has a one year warranty including parts and labor. When the LINK ELECTRONICS warranty period has expired, the product will be repaired at $90.00/hour, plus parts. The RA number is valid for only 30 days from the date issued. A new RA number is required if period exceeds 30 days. The RA number is valid only for items originally listed on the RA requested form. The items are tested only for the problems listed in the RA form, so please describe the problem clearly. “Dead on Arrival” must be reported within 72 hours of receipt. Any physical damage or rework done to the product or defacing labels on parts & components voids the warranty. Please be sure to include all accessories items and hardware in its original condition/packing. Please print RA number legibly on top of the package, and on the inside of package with RA. All shortcomings, discrepancies, short-shipment etc. should be brought to the notice of Link Electronics within a period of 2 days from the date of the receipt; Link Electronics will not be responsible, or liable thereafter in respect of this RA.
